Service agreement and compliance guidelines for U.S. healthcare providers
Welcome to MedCore RCM. These Terms of Service govern your use of our medical billing and revenue cycle management services. By engaging our services, you agree to these terms and all applicable U.S. healthcare regulations. Please read this agreement carefully before proceeding.
SECTION 1: Service Description
MedCore RCM provides end-to-end revenue cycle management services to U.S. clinics, physician practices, and hospitals from our operations center in Jaipur, Rajasthan, India. Our services are strictly administrative in nature and do not include clinical care, medical advice, or patient treatment of any kind.
Core services covered under this agreement:
- - Medical billing and claims submission
- - Denial management and appeals
- - Accounts receivable (AR) follow-up
- - Provider credentialing and enrollment
- - Payment posting and reconciliation
- - EHR/EMR data review and support
- - Patient statements and collections
- - Revenue reporting and analytics
SECTION 2: Client Responsibilities
To receive our services, the client (healthcare provider) agrees to:
- - Provide accurate and complete patient demographic and insurance information
- - Maintain valid medical licenses and provider credentials
- - Ensure clinical documentation adequately supports all billing activities
- - Notify MedCore RCM promptly of any payer contract changes or credentialing updates
- - Comply with all applicable federal and state healthcare billing regulations
- - Designate a point of contact for day-to-day communication
- - Respond to information requests within 3 business days to avoid billing delays
MedCore RCM is not responsible for billing delays or claim denials caused by inaccurate, incomplete, or late information provided by the client.
SECTION 3: HIPAA Compliance and Business Associate Agreement
MedCore RCM operates as a Business Associate under the Health Insurance Portability and Accountability Act (HIPAA). Before any Protected Health Information (PHI) is shared or accessed, both parties must execute a signed Business Associate Agreement (BAA).

Our HIPAA safeguards include:
Administrative safeguards:
- - Staff HIPAA training and access controls
- - Incident response and breach notification procedures
- - Regular internal compliance reviews
Physical safeguards:
- - Restricted facility access
- - Secure workstation and device management
Technical safeguards:
- - 256-bit SSL/TLS encryption for all data in transit
- - Multi-factor authentication for system access
- - Audit logs and access monitoring
- - Secure data storage and transmission protocols
In the event of a suspected data breach involving PHI, MedCore RCM will notify the affected client within 60 days as required by HIPAA's Breach Notification Rule.
SECTION 4: Performance Standards
MedCore RCM commits to the following service standards:
- - Claims submitted within 48 hours of receiving complete documentation
- - 96.8% clean claims rate across all payers and specialties
- - Denial responses initiated within 24 hours of receipt
- - Monthly performance reports delivered by the 10th of each month
- - Dedicated account manager assigned to every client
- - Email response to all client inquiries within 24 business hours
These standards apply under normal operating conditions. Performance may be affected by incomplete documentation, payer system outages, or circumstances outside our reasonable control.
SECTION 5: Fees and Payment Terms
Service fees are structured as either:
(a) A percentage of monthly collections, or
(b) A fixed monthly fee based on practice size and service scope
Specific fees are outlined in each client's individual Service Agreement.
Standard payment terms:
- - Invoices issued monthly with itemized reporting
- - Payment due within 30 days of invoice date
- - Late payments may incur a 1.5% monthly interest charge
- - No hidden fees — all costs disclosed in the Service Agreement
SECTION 6: Confidentiality
Both parties agree to maintain strict confidentiality of all proprietary business information, patient data, and operational details shared during the course of this engagement. This obligation survives termination of services.
MedCore RCM will not sell, share, or disclose any client or patient information to third parties except as required to perform contracted services or as mandated by applicable law.
SECTION 7: Limitation of Liability
MedCore RCM's services are limited to administrative billing functions. We do not provide clinical, legal, or financial advisory services.
Our total liability for any claim arising under this agreement is limited to the total fees paid by the client to MedCore RCM in the 12 months preceding the claim. We are not liable for indirect, incidental, or consequential damages.
SECTION 8: Termination
Either party may terminate this agreement with 30 days written notice sent to the other party's designated email address.
Upon termination, MedCore RCM will:
- - Complete all claims in progress at the time of notice
- - Provide a final performance report and reconciliation
- - Transfer all client records and billing data within 15 business days
- - Securely return or destroy all PHI in accordance with HIPAA requirements
- - Provide a written certificate of data destruction upon request
SECTION 9: Governing Law
This agreement is governed by applicable U.S. federal healthcare regulations including HIPAA, CMS billing guidelines, and relevant state regulations in the client's state of practice. Disputes will first be addressed through good-faith negotiation between both parties.
